b13da28587 implement a bunch of stuff: 
* column separator (instead of global column prefix)
* per column suffix
* indexing columns by id-number instead of column header
* optional header supression (via settable flags)
* ability to end a row
* don't extend last column to full width

2e20f6c28f check return values from ldap operations and close it we get back 
LDAP_SERVER_DOWN. stupid ldap client lib, you should retry by yourself.
